      Facilitating One Page Profiles and Person Centered Reviews

      Using person centered practices to develop and review support plans A person-centered review uses person-centered thinking tools to explore what is important to someone now and in the future. It looks at what support they need and what is working and not working from different perspectives to determine desired outcomes and actions for change. This program is great for anyone interested in helping to develop person centered plans. You must attend all six required sessions to receive a certificate of completion.  All sessions will be from 9am to 4pm. Fall Program schedule This program is open to independent facilitators, service providers, people who want to facilitate their own person-centered plans, friends and family members, and regional center staff. Enrollment is up to 8 people per series. You can check for the next public offering or contact Mary Beth to bring this program to your organization or community. Your program facilitator: Mary Beth Lepkowsky, Helen Sanderson Associates USA, marybeth@helensandersonassociates.com Things to consider: All participants must have completed Person Centered Thinking training (12-hours) within last three years. All participants will be supported to facilitate a person centered review while being observed and will receive coaching and feedback. Participants are expected to attend all sessions and complete all assignments to receive a Certificate of Completion. When selecting participants we will seek a diverse group of regional center staff, service providers, and individuals who receive supports and family members to create opportunities for partnership. Each participant will receve individual coaching and peer feedback throughout the coure series Participants will receive a resource pack of facilitator guides and materials to support the planning process. Participants will have access to an online portal where you can download up to date resources and facilitator materials. 2019-2020 Person Centered Reviews & One Page Profiles Program Schedule Attendance at all sessions is required to receive a certificate of completion. Session 1: November 16 – 9am to 4pm The Powerful Practice of Person Centered Reviews & One Page Profiles This session provides an overview of person centered planning and Person Centered Reviews. You will explore when and how to use the person centered review process and how to use person centered thinking skills to discover information about what matters most to the person receiving support. You will practice developing one page profiles and perform a quality check to make sure they are both meaningful and useful. Session 2: November 23 – 9am to 4pm Using Person Centered Practices to Discover What Matters Now and in the Future Learn the art of discovery by practicing person centered skills such as relationship map, communication charts, decision making agreements, and hopes and dreams for the future. Create a person-centered description during an “Important to and for” Review. Session 3: December 14 – 9am to 4pm Preparing for and Facilitating a Person-Centered Review You will learn five essential steps to prepare for a Person Centered Review and discuss the strategies needed to make facilitation work most effectively. You’ll apply best practices and new techniques using video-based examples and role playing scenarios. Session 4: January 11 – 9am to 4pm Developing Meaningful Outcomes and Person-Centered Goals You’ll learn to use information gathered from a Person-Centered Review to develop SMART- specific, measurable, attainable, relevant and time limited goals and outcomes. When done well, person centered goals and action plans provide guidance about how to support the person to maintain personal choice and control while balancing the right level of support. Practice these skills during a “Working/Not Working” Review. Sessions 5 & 6: January 25 & February 1* 9am to 4pm (the time and date of these sessions may be customized for each participant) Putting Person-Centered Reviews into Practice Each program participant will facilitate a Person-Centered Review with a person you support or a team at work. You will receive individual feedback on your preparation and feedback. This final session will focus on sharing collective insights and reflective learning, discussing common facilitation challenges, and exploring ways to incorporate Person Centered Reviews in your future work. Stay connected!   Facilitators who complete this course will have continued access to an online site where you can access the most up to date versions of facilitator resources and materials to use with individuals, families, and invited guests.
